Add Price to Text Input

Is there a way to add a price field to the “Text Input”. My site offers custom apparel and I need an option for customers to add a Name or Number to their purchase. It’s only an option, so I need to charge more if they choose to add personalize a product.

I would put a radio with price before the text input.
Make tekst input conditional on selecting Radio
